The Live Healthy Little Havana Host Board of Directors, with the support of the Host Agency, guides the participatory planning process and will establish a foundation for fundamental, multi-sector, neighborhood-wide changes that significantly improve the long term health and quality of life of local residents. For each Health Impact Area, a Health Impact Team (HIT) has been formed. HIT members will bring to bear their diverse and extensive expertise to ensure the success and growth of Live Healthy Little Havana.

Live Healthy Little Havana is now focused on the Social determinants of health. Social determinants of health are conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. As a result, Immigration, Law Enforcement, Employment, Education, Health Care, and Public Realm are the main areas of focus for the initiative.
